Upon completion of a medical history form, a health record is automatically prepared for you and placed on file in the Student Health Center. Your record is kept at the Student Health Center for seven years after the last entry date, then destroyed. Your medical record is not part of the University record system and is not included in educational records. If you want a copy of your medical record or immunizations when you leave school, you must request it from the Student Health Center.   It is strictly confidential. Information from your medical record will not be released without your written permission except in circumstances of danger to yourself or to others unless otherwise provided by law.

If you want a copy of the immunization record you submitted to UNC, you need to request that from the Health Services/Insurance Office located in the Student Insurance Office in the second floor of Cassidy Hall (phone: 970-351-1919 or 351-1915).
